Acting under delegated authority from the Minister of Health, I, John Crawshaw:

1. revoke the approval of the Nelson Marlborough Addictions Service and medical practitioner issued pursuant to section 24(7)(a), (b) of the Misuse of Drugs Act 1975 on 7 March 2014 (as published in the New Zealand Gazette, 13 March 2014, No. 29, page 831); and
2. specify the Nelson Marlborough Addictions Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, at the following places:
   1. 31 Pascoe Street
      Tahunanui

      Nelson 7011

   2. 31 Hospital Road
      Blenheim 7201

   3. 18 Tudor Street
      Motueka 7120

   4. 34 Motopipi Street
      Takaka 7110; and

3. pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, specify Dr Michael James Haskew, as a medical practitioner, who may:
   1. prescribe, administer and supply controlled drugs for the treatment of dependence, as the lead clinician for Nelson Marlborough Addictions Service;
   2. authorise in writing service/clinic medical practitioners as specified under section 24(2)(b) of the Misuse of Drugs Act 1975 to:
      1. prescribe, administer or supply controlled drugs for the treatment of dependence:
      2. authorise in writing general practitioners receiving clients from Nelson Marlborough Addictions Service as specified under section 24(2)(d) of the Misuse of Drugs Act 1975; and
   3. authorise in writing general practitioners receiving clients from Nelson Marlborough Addictions Service as specified under section 24(2)(d) of the Misuse of Drugs Act 1975.

Conditions

1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Nelson Marlborough Addictions Service.
2. This approval expires on 31 December 2020.
