Pursuant to the New Zealand Public Health and Disability Act 2000 and the Crown Entities Act 2004, I have appointed

as a member and chairperson of the Health Quality and Safety Commission board.
I have also appointed

as members of the Health Quality and Safety Commission board.
These appointments commence on 1 December 2010 and end on 30 November 2013.
