Pursuant to sections 126 and 127 of the Medical Practitioners Act 1995, the council gives notice that the following fees (G.S.T. included) are payable effective from 1 April 2001:
A. REGISTRATION FEES
On application for registration $150.00 5
For initial admission or restoration to interim, probationary or general registration $225.00 1 & 5
For temporary registration for the purpose of:
? Giving postgraduate instruction (class 1) Nil
? Obtaining postgraduate training or experience or carrying out research (class 2) $710.00 2 & 3
? Providing services in special circumstances approved by the Council (class 3) $710.00 2 & 3
For vocational registration:
? Interview $1,000.00
? NZSPEX Written and/or Clinical Evaluation maximum $3,050.00
For admission to vocational registration $198.00
For provision of the following certificates:
Interim or extension $33.00
Registration (or copy) $33.00
Standing (or copy) $33.00
Any other document certifying registration status or standing $33.00
B. ANNUAL PRACTISING CERTIFICATE (APC) OR INTERIM APC INCLUDING DISCIPLINARY LEVY
(i) Fee for 12 month APC or interim APC (including levy, in brackets) $485.00 ($125) 4
(ii) Quarterly APC/levy to bring medical practitioners recommencing practice during practising year into cycle
(iii) Duplicate APC $33.00
C. DISCIPLINARY LEVY-levy for 12 months $125.00
D. CONTRIBUTION TO MENTORING COSTS-(18 month programme) $2,025.00
E. CONTRIBUTION TO EDUCATIONAL SUPERVISION COSTS
(12 month programme; assumes 12 sessions) $2,160.00
F. NEW ZEALAND REGISTRATION EXAMINATION (NZREX)
Whole NZREX Clinical (and repeat) $2,560.00
Repeat of separate disciplines: Long case $1,250.00
